Your first visit to our practice establishes the groundwork for our professional relationship. Upon arriving, we will ask you to provide some pertinent information so we may get to know you, including your medical and dental history and the reason you've come to our practice.
The information on these pages is intended to help you understand what to expect for your first visit. You’ll also find information, such as directions to our office, practice hours, payment policies and more. There is also information about Dr. Bond and our team and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms listed below in advance of your appointment.
Being well-prepared for your appointment will ensure that we have all of the necessary information to provide you the best possible care. This includes bringing your physician’s information, a list of any medications you may be taking, and your dental insurance information, which includes the name, address and phone number of the insurance company and group number as well as your dental insurance card if available.

Initial Oral Examination
Your initial examination includes a visual examination, charting, periodontal probing, diagnosis and treatment recommendations. We will also take any necessary x-rays for proper diagnosis of decay and other oral conditions.
If for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

If you have an after-hours emergency you can call (817) 668-6076 to leave a message for Dr. Bond.
Some examples of dental emergencies are:
A broken or knocked-out tooth
Sever toothache or jaw pain
Pain when chewing or to heat/cold
Bleeding from a particular area of the mouth
Sudden change in tooth color
Visible pus from infection around the gum area

Insurance and Billing
As a courtesy to our patients, our insurance coordinator will prepare and submit insurance documentation. As a courtesy we will accept benefit assignment and prepare an estimate of expected payment. The patient is responsible for payment of his or her estimated payment portion at the time of service. Since it is impossible for this office to guarantee insurance payments, the patient is responsible for any balances not paid for by his or her insurance company.
We accept cash, checks, debit or credit cards (Visa, MasterCard American Express). All deductibles, non-covered services and patient portion co-payments are due the day services are rendered unless prior financial arrangements are made.
To make things easier for you, we are pleased to offer CareCredit, a GE Money Company, to help you pay for the treatment you need to get the smile you deserve. Several plans are available, including No Interest financing or low minimum monthly payment options so you can get what you want, when you want it.
